Care Management / In Home Services
Even frail elders can remain at home and independent if they have assistance. Elder Services' Care Management Program provides that assistance.
A Care Manager visits the elder at home to discuss the elder's needs. At this initial assessment, available services are reviewed with the elder and a care plan is developed. Services can include homemaking, laundry, meal preparation, transportation, shopping, companionship, friendly visitors, money management and personal care.
The Care Manager implements the care plan by hiring appropriate staff and monitors the plan by making regular home visits and telephone contacts. The Care manager handles any staffing problems and acts as an advocate for the elder. If an elder is income eligible and meets frailty guidelines, Elder Services can provide financial help to offset some of the cost of in-home services. Some services have no cost, some require a co-payment or suggested donation; some call for a full fee.
